Department/School request for short term exam arrangements

  • Important information:
    This form is to be completed by department/schools on behalf of any student who requires adjustment(s) to his or her examinations due to an accident or a short term illness which only affects the upcoming exam period. Under no circumstance should this form be completed by a student. The deadline to submit your request for the January exam period is 5th January 2018.     Please indicate below which adjustments are required for the student's exam(s). You must select at least 1 arrangement. If you require more than 4 arrangements or have a special requirement, please contact the Exams Office. Please note that the arrangements selected below will be applied to all exam modules which have been listed in the previous section. Please note: If a student is moved to an AEA location they will automatically be in a room of 25 or less.
  • If you have selected rest break arrangements above, please indicate what the student is permitted to do during his/her rest breaks
  • I can confirm that this form is not being used as an interim arrangement for a long term medical condition. (if you cannot confirm this, please do not submit this form)
