Induction Procedures and Questionnaire
The health and safety induction proceedures for new recruits have been reviewed by the University Safety Committee. It is considered that the provision of basic,general health and safety information at a very early stage of a new recruit's time at the University is essential. All Departments are reminded of this important matter and to assist in the provision of this, and to promote consistancy across the University, a Health and Safety Induction Questionaire has been developed.
The purpose behind the questinaire is to make the new recruit find out basic safety information thus raising awareness and consolidating this. A passive process whereby someone is merely given safety literature to read is much less likely to achieve this.
The preferred method for ensuring that appropriate health and safety information is given to the new starter on induction is that s/he completes the attached questionnaire within a short time of starting in the Department. The individual should be given the questionaire on arrival at the Department and their manager, supervisor, or Departmental Safety Officer (whoever is designated by the Department) ensures that they have enough knowledge to complete the questionnaire satisfactorily. The questionaire should be completed within the first week and returned to the designated person prior to incorporation into their personal records.
It is noted that some departments will not need all parts of the questionnaire and therefore it is expected that Departments will modify the questionnaire to suit their needs. The questionnaire is available from the Safety Office in an electronic format to assist in this. A WordPerfect 5.1 or 6.1 version will be forwarded via email to DSOs, on request, for easy alteration.
If a department feels that the induction procedures they already have in place are adequate it is not compulsory to use the questionnaire system. However, whichever system is used this should be described in the Departmental Safety Policy.

This questionnaire is designed to ensure that you have understood all the basic safety rules and procedures of the school/department you work in. You should be able to complete all sections. On completion, arrange to discuss your responses with your section manager or School/Departmental Safety Officer.
Information and procedures associated with issues such as radiation, hazardous substances, etc which may be specific to your school/department must be provided to you by the school/department. You have a responsibility to ensure you are fully informed before starting work.
