Cold agglutinins cause RBC clumping at temperatures below 37C (for example, at room temperature of 22C). This can result in an artifactual reduction of the RBC count, with increased MCHC and increased MCV. Warming the sample to body temperature reverses these changes.
Cold agglutinins may occur in lymphomas, auto-immune disorders, and with certain infections (infectious mononucleosis and severe Mycoplasma infection).

Anisocytosis on blood smear correlates with RDW of automated counter. However, the counter does not measure shapes. Therefore, the striking poikilocytosis on the blood smear appears more severe than the increase in RDW would suggest.
