GRASP-AF is a free, easy-to-use tool that assists GP practices to interrogate their clinical data enabling them to improve the management and care of patients with atrial fibrillation (AF) and to reduce their risk of stroke through appropriate intervention with anticoagulation. The tool also assists with case finding activity, helping practices to establish more accurate prevalence rates within the practice population.
The GRASP-AF quality improvement tool is freely available to all PRIMIS Hub members within England.
Obtaining help and information
Any CCG (only) staff who require access to the CCG comparative report in CHART Online for any PRIMIS' tools should complete and submit the form on the following page:

This tool includes the option to upload data to CHART Online. This allows GP practices and CCGs to compare and benchmark practice data both locally and nationally.
Nene CCG GRASP-AF case study
The GRASP-AF quality improvement tool has played an important part in a Nene CCG stroke reduction initiative that has seen emergency hospital admissions fall. The project’s success in improving the management of patients at high risk of stroke led to its developer, Dr Yassir Javaid, being named as GP of the year 2015 by PULSE:
