6 Patient-reported work-relevant difficulty, recommendations and goals:

Difficulty Recommendations / goals
Worried about sickness absence and build up of email correspondence. It would be helpful if his employer could clarify the situation about his sickness absence record, and offer support in managing his inbox.
Has difficulty concentrating and making decisions To return to work 3 days per week before end of academic year, gradually picking up exisiting/familiar work over the student summer break.
Low mood, symptoms worse in morning, especially while getting used to new medication. To work 11- 4 initially. May need support with allocated parking to ensure place available, until he has resumed usual hours.
Anxious about facing groups of students. To start limited teaching duties from September, e.g. 3 hours per week initially, with topics/students he feels most comfortable/confident with.

Mr Prasad is attending our unit for a six-week course of treatment (Weds ams 10-11). He has a 'not fit' fit note which expires on 14th June. He is ready to start a phased return to work as soon as the above recommendations can be addressed. I will review his progress on 1st August. I estimate he will have returned to full hours and duties by mid November.
