Aims

The overall aim is to evaluate the End of Life Long terms Conditions service being piloted. The service endeavours to provide evidence-based generalist palliative care in the community through direct clinical support to patients with long terms conditions who have generalist palliative care needs. It also provides support to services that offer generalist palliative care to patients. Specifically the evaluation endeavours:

  1. To determine whether the pilot is meeting its aims and objectives
  2. To determine the successes of the pilot
  3. To determine barriers to success
  4. To determine patient, provider (as referrers) and staff (from provider) experience
  5. To evaluate how well the pilot operates within the wider care pathway and any unintended consequences
  6. To identify any issues that impact on service delivery
  7. To make recommendations around existing and future service delivery

Methods

Mixed methods approach using both implementation evaluation and outcomes evaluation.

Stage of Development

The project is due to start in July 2015.
