I have a question about the ongoing achievement record. How many pieces of evidence is a student expected to present for each competency?
It is down to the mentor to decide how much evidence is acceptable as the mentor needs to ensure that the student has achieved their competency consistency in a variety of ways.
 
I am currently a sign off mentor. However I last attended a mentor update two years ago and have been sent something about a triennial review. Do I need to do anything?
Yes, every mentor needs a yearly update one of which must be face to face. You need to contact your Placement Learning Support Unit who will be able to advise when the next training days are due to take place. In terms of the triennial review, all mentors had have a triennial review with their trust manager every three years. To reach the requirements of the triennial review you must mentor two students in three years, have an annual mentor update and complete the triennial review.
