How to obtain a replacement University Card
If your card has been lost, stolen, damaged, is faulty or has incorrect data on it then you may request a replacement. For lost, stolen and damaged cards there is a £15.00 replacement fee. The replacement fee for stolen cards may be waved at the discretion of the Head of Security if a crime number is supplied.
Please also remember that if your card has been lost or stolen then it is your responsibility under the implied terms and conditions of University card use that you report your card as lost or stolen to the University card office on 0115-9515759 or by emailing email@example.com immediately. This will ensure that your lost or stolen card can be blocked and prevent unauthorised use.
Replacement University Card Request Form (Word)
To obtain a replacement you will need to complete the 'Replacement University Card Request Form' above and take it to the Security office, rear of the Hallward Library, University Park, Nottingham, NG7 2RD, together with the replacement card fee (if applicable). For staff / students that do not work within close vicinity of University Park then contact the University card support office to make alternative arrangements.
If you have never before held a University Card then you will need to make your first card application.
