Giving practices the edge for QOF: save time, improve care and maximise QOF points

The new PRIMIS Casefinder audit tool has a data quality focus allowing practices to efficiently identify patients who may not have an appropriate diagnosis code. As well as saving time and increasing the care provided to patients, practices will also be able to increase QOF points by identifying patients that should be added to a QOF disease register.

This PRIMIS Casefinder audit tool contains the following three* query sets, which are all included within the QOF:

  • Dementia - this Casefinder topic will help practices identify patients with unrecorded dementia which will be of particular help in preparation for the upcoming Direct Enhanced Services (DES)
  • Osteoporosis - this Casefinder topic will help pinpoint any patients with missed fragility fractures and those with undiagnosed osteoporosis, in particular helping the practice achieve QOF OST1 and OST2 indicators
  • Peripheral Arterial Disease (PAD) - this Casefinder assists practices in looking for unrecognised patients with PAD as well as defining their QOF target population

The Casefinder audit tool is available to download via the Update facility within CHART - if you are a full member the casefinder file will be available for you the next time you check for updates.
