CCGs working with practices on diabetes programmes can now request access to view comparative reports in CHART Online - a timely resource to help implement STPs

CHART Online CCG comparative reports for the Diabetes Care quality improvement tool are now available on application. These reports allow CCGs to compare outcomes across all their pratices (that have contributed data) and against CCG and national averages.

This new comparative availability for CCGs is timely as localities begin to implement their STPs.The three headline issues of improving quality, improving health and wellbeing and improving efficiency of services can all be supported through the use of PRIMIS' quality improvement tools.

These detailed reports give an exceptionally useful view of the overall picture within the CCG (from contributed data). This includes synopses and graphs (where appropriate) for CCG prevalence, key performance indicators, care processes done (the eight care processes) and achievement against targets separated for Type and non-Type 1 Diabetes for HbA1c levels, blood pressure and cholesterol.

An overview of practices' data uploads by date are also tabled (including a list of those still to upload) so CCGs can monitor activity levels for their diabetes programme.

CCGs and localities may wish to promote the use of the free Diabetes Care quality improvement tool and the upload of results to CHART Online by practices in their area to improve the effectiveness and completeness of the reports. Please contact PRIMIS if you would like us to help improve uptake in your locality or require additional imformation.

In addition to Diabetes Care, we are curently working towards making CCG comparative reports available for the Asthma Care quality improvement tool.


How to obtain access to view CCG comparative reports

CCG staff requiring access need to fill in a short online form (see link below) for PRIMIS to assess and then grant access, as appropriate. Please note that only NHS email addresses will be accepted in the request.

Request access

Any CCG staff member requesting this access wwill need to be a PRIMIS Hub member in order to obtain CHART, so CHART Online can be used. Sign up for a free Hub membership.

CCG STAFF who already are CHART users still need to request this access to view the Diabetes Care toolkit for their practices.

Please note practice staff using CHART do NOT need to fill in this form - please just carry on using the tools as usual.


The development of the Diabetes Care quality improvement tool has been funded by Boehringer Ingelheim. Boehringer Ingelheim has undertaken a medico legal review but PRIMIS has retained editorial control and intellectual property rights for this audit tool.

PRIMIS does not share data with the pharmaceutical industry.
