An update to the Diabetes Care quality improvement tool is now available for all PRIMIS Hub members.

This version incorporates the recent changes to NICE guidance (released December 2015) and its blood glucose lowering pathway, which will help practices and CCGs measure their patient care against these national guidelines.

This update also includes the ability to upload to CHART Online which means that the results prepared by the tool can be used for benchmarking and comparative analysis purposes.

  • CHART Online is an important additional step to view a practice's position and track progress over time against other practices within the same CCG or nationally
  • A 'call to action' report is generated and accessed in CHART Online, once data is uploaded, which will highlight any areas of interest or concern to the practice
  • CCGs can access key reports on the progress and outcomes of the practices within their locality

Key new features in the Diabetes Care quality improvement tool latest version:

  • changes to NICE guidance, specifically around the intensification of drug treatment and the blood glucose lowering pathway
  • updated filters in the casefinder to support the Public Health England Diabetes Prevention Programme (NHS DPP) to help organisations that are part of the NHS DPP pilot as it is an invaluable reporting tool
  • new functionality allowing direct access from the summary sheet to patient lists in both the case finder and the main care audit
  • 'click through' from the blood glucose lowering pathway directly to lists of patients, including those considered 'out of range'
  • additional graphs which provide further flexibility in accessing lists of patients worthy of further investigation

Obtaining the Diabetes Care quality improvement tool:

All Hub members (whether basic or full subscription) can immediately access this new version of Diabetes Care via 'Check for updates' within CHART

Diabetes Care and the NHS Diabetes Prevention Programme (NHS DPP)

This new release of Diabetes Care is just the first step in PRIMIS' ongoing improvement to this tool for 2016, ensuring that it stays updated to code releases, NICE guidance developments and national programmes. Some further changes to the casefinder element and a new version will be made available in May to reflect new DPP coding concepts due within the last ever Read code update (April 2016).
