The latest release of the free Diabetes Care quality improvement tool is now available for download. [Go to ‘Check for updates’ within CHART. Latest version is 2.2.0]

This latest version concentrates on improvements to the case finder element of the tool and includes a dedicated NHS Diabetes Prevention Programme (NHS DPP) summary sheet.


As part of our commitment to continually improve this tool throughout 2016, three previous enhancements to Diabetes Care have been released already this year.

The features within Diabetes Care make it an excellent choice for ongoing monitoring of both the NHS DPP and the progress of at-risk patients. Plus, the tool can still be used to identify patient patients for the NHS DPP.

Key features and changes in this latest version [2.2.0] include:

  • filters those patients most likely to be eligible for the NHS DPP
  • provides a list of 'at-risk' patients who do not have a glycaemic assessment
  • provides a breakdown of glycaemic assessment status using the latest HbA1c/fasting blood sugar results
  • provides a mail merge list of patients requiring a glycaemic assessment
  • provides a mail merge list of patients with glycaemic assessment suggesting eligibility for the NHS DPP
  • provides a summary of all NHS DPP activity recorded by the practice
