PRIMIS launches the Diabetes Care audit tool

PRIMIS is delighted to launch its new Diabetes Care audit tool, which is free to use for all GP practices in England.

The Diabetes Care audit tool has been designed to help practices audit their clinical data, helping them to optimise the management and care of patients with diabetes and reduce their risk of developing associated complications.

The Diabetes Care audit tool is a comprehensive, easy-to-use tool with both case finder and a patient management element. The case finder helps GPs to identify patients most at risk of developing the disease through assessment of key risk factors, including use of the Leicester Diabetes Risk Score and creates a profile of patients with a BMI over 25.  The case finder also identifies patients who may have a missing diagnosis of diabetes, enabling practices to build and maintain an accurate disease register.

The management element looks at all patients with a recording of diabetes, regardless of type, identifying areas for improvement and where the risk of developing associated complications might be reduced, as well as helping practices to easily audit patient care against current best practice.

Both practices and CCGs can utilise the comparative analysis feature of the audit tool for benchmarking purposes.  By uploading the data securely and anonymously to CHART Online results can be compared at local or national level.

The audit tool has been funded by a grant from the Boehringer Ingelheim and Lilly Diabetes Alliance.

Using the Diabetes Care audit tool

The audit tool uses PRIMIS’ CHART software to create highly visual practice-based summaries including a dashboard and pre-populated NICE medication pathway, plus the ability to drill down and examine detailed patient care at an individual level.

As well as being a tool to help practices improve management of patients with diabetes and identify potentially unrecorded or undiagnosed patients, the tool also contributes to the delivery of the QOF, the National Diabetes Audit, the NHS Outcomes Framework and the CCG Outcomes Indicator Set.

Availability

The Diabetes Care audit tool is free to use for any practice that is a member of the PRIMIS Hub membership scheme, including free basic membership.

Learn how best to use the Diabetes Care audit tool at the PRIMIS Forum

The Diabetes Care audit tool will be featured at the PRIMIS Forum on 18th September in Nottingham.  A PRIMIS Clinical Adviser session will give practices key advice to make the most out of the tool’s use and interpret the results data.

There will also be a PRIMIS helpdesk stand which you can approach with any audit tool or CHART queries during the event.

The Forum is a training event that aims to bring together all those who are interested in the better use of healthcare data.
