The latest version of the Diabetes Care quality improvement tool now has easy-to-read practice reports, which are linked to CHART Online comparative data. These printable summaries offer additional narrative reporting and informative calls to action for practices.

The Diabetes Care quality improvement tool helps practices optimise the management and care of patients with diabetes and reduce their risk of developing associated complications.

After the quality improvement tool has been run, results can be uploaded to CHART Online for comparative analysis with other practices within a CCG area (that have also uploaded data), and from all practices nationally. The new summary reports are available from CHART Online.

The reports cover:

  • prevalence of the different types of diabetes in the practice’s disease register
  • two key performance indicators (percentage of patients receiving all eight of the nine processes for diabetes undertaken in primary care [retinal screening is completed in secondary care] and of those patients receiving all eight those which also achieve target ranges for HbA1c, blood pressure and cholesterol levels)
  • cholesterol targets
  • diabetes medication safety summary

Using CHART Online and accessing practice reports with Diabetes Care

In order to make use of the narrative reports, practices must already have data that has been uploaded to CHART Online from the Diabetes Care quality improvement tool (60 practices have already uploaded data).

Once sufficient data has been uploaded by a practice for comparative analysis, then they will be able to produce a practice report. Practices do need to log in to Hub to access CHART Online.
