Warfarin Patient Safety now has full CHART Online functionality, enabling practices to upload data for benchmarking and comparison, as results accumulate.

Earlier this year, an upgrade to Warfarin Patient Safety was released featuring significant improvements including adaptation to CHART Online. Now, the CHART Online secure database has been configured so that practices (who have already downloaded and begun using this audit tool) can upload their Warfarin Patient Safety results.

CHART Online enables reporting and displays for benchmarking and comparison via a secure online database that stores pseudonymised audit tool results, such as those from Warfarin Patient Safety.

CHART Online works most effectively for comparison once there is a significant amount of data available to make meaningful analysis and comparison between practices and peers. Once the CHART Online database has been populated with sufficient data, then the comparison functionality can be made available to others.

PRIMIS would like to encourage any practice who has downloaded and used Warfarin Patient Safety since its last release (version 1.12 July 2015) to begin uploading their results.

What does the audit do?

The Warfarin Patient Safety audit tool enables practices to audit their clinical data to look at the appropriateness of warfarin prescribing. In particular, the tool assists in examining whether patients are achieving the optimum benefit from taking warfarin by calculating the amount of time their International Normalised Ratio (INR) is within therapeutic range (TTR).

The recent upgrade to the tool included the following improvements:

  • calculations in the data sheet now feature CHA2DS2VASc, which has replaced CHADS2 to reflect changes made in the Quality and Outcomes Framework (QOF) in regards to atrial fibrillation
  • results in the summary sheet now highlight percentage time in therapeutic range (TTR) using a 65% value, rather than 70%. This change was made to reflect the NICE recommendation that any patient with a TTR below 65% should have anticoagulation reassessed. Development of the tool

The development of the Warfarin Patient Safety audit tool has been funded by Boehringer Ingelheim. Boehringer Ingelheim has undertaken a medico legal review but PRIMIS has retained editorial control and intellectual property right for this audit tool.

How to obtain Warfarin Patient Safety

Warfarin Patient Safety is an entirely free-to-use audit tool. All that is required to obtain the audit tool is a free basic PRIMIS Hub membership and the software tool, CHART, which is also available without charge from PRIMIS.

All PRIMIS Hub members can obtain Warfarin Patient Safety by using the ‘Check for updates’ link in CHART.
